본인의 이름과 연락처를 남기고 싶으면 남긴다.

추가로 기관에서 연락을 하고 싶은경우 어떻게 연락받고 싶은지를 정한다. Email 옵션이 무난한다. 이름과 주소는 안적어도 이메일 주소는 꼭 적는다.
